Maintenance record entries
Logbook entries for airframe, engine, propeller and appliance records with description of work, date, aircraft times, and the name, signature and certificate number of the person approving return to service. Inspection entries include the required 'in accordance with' statement and airworthy/unairworthy determination.

Each discrepancy has its corrective action and the manual reference used. That text is the description of work.

Sign-off by the right person
The inspector enters the RTS statement; their name and certificate number are on the record. Only inspectors can do this.

Return-to-service statement
I certify that this aircraft has been inspected in accordance with an annual inspection and was determined to be in airworthy condition.
